Unable to create a new record when custom access rights are applied

I have setup the User Rights by Record, and it looks all good. However during testing when the test user was creating a new record, after the custom user rights were applied (through the User Interface provided), the record opens displaying the events / forms, we try and click on a form to start editing and the screen goes back to the record status dashboard, and does not create the record. If the custom user rights are are removed from the user they are able to create a new record without any problems. Has anyone come across this issue? I am fairly new with REDCap.
