This PR unvendors the pipeline, using the code from LensKit instead of a copy maintained and synchronized here.
The LensKit pipeline docs now also have a section explicitly describing how the pipeline is decoupled from any particular data structures, and that POPROX uses the pipeline without LensKit's models or data structures.
