When creating a GitHub issue, the text is pre-populated with a template, which asks the submitter to include relevant information. Unfortunately, the template has issues that limit its effectiveness: it is not well organized (e.g. redundant requests for version numbers) and is rather off-putting (e.g. "FAILURE TO PROVIDE THIS MAY RESULT IN YOUR ISSUE BEING IGNORED"). This makes it more difficult than needed to submit a complete bug report, and increases the chances that the template will be ignored completely.
Replace this with a GitHub issue form which requests the same information, but is easier to read and allows inputs to be validated.
