New features to support the retirement of the DF Pilot application:

Added "business_guid" and "display_name" fields to initial DB user creation step
Added new notification banner with pre-retirement message
Added new notification banner with post-retirement message
Added logic to disable creation of new forms post-retirement
Added logic to disable the editing of a previously started form post-retirement
Reduced number of offline ticket numbers to request from 10 to 3
Updated ILO list with latest changes for Prince Rupert, Terrace, and Kitimat
