This adds a new annex where the conventions are documented. There are still more details to be added, including the format of the metadata file itself

This PR incorporates P2577R2, and leaves the space for the future work. This addresses part of #5, but there's still more work to be done there, particularly the format of the metadata files and the specific convention for the translation of linker input files to module metadata files.
