Plugin Version 0.4 for MantisBT-2.2

1. Renamed the plugin to SuggestUsername
2. Added Mantis-jQueryUI plugin as a prerequisite.
3. Removed jquery-ui files
4. Used EVENT_LAYOUT_BODY_END instead of EVENT_VIEW_BUG_EXTRA hook
5. Updated documentation
