Change generated entity ids to support multiple devices.

Note
If you end up with one device that only has an unavailable entity after the update, please try to reinstall the integration using the GUI within home-assistant.
To support multiple devices the internally generated identifiers had to be adapted. HA may not delete those previous devices from its list, therefore leaving too many devices.
