OSHRTT-18 Adding EOM test patient data

Here is my first attempt at a patient record with all the EOM fields we discussed. I based it off the Ancora synthetic patient discussed in the JIRA ticket and I just left in all the other extra resources in the bundle, I can strip them out if we think that is better. For review, I think it would be good to have someone double check that I actually have all the fields we need and that it's all proper valid FHIR.

Pull requests into the clinical-trial-matching-service require the following. Submitter should ✅ when done. For items that are not-applicable, note it's not-applicable ("N/A") and ✅.

• Make sure test coverage didn’t decrease. If you are allowing the test coverage to drop, leave an explanation as to why:
• Does an update need to be made to the documentation with these changes?
• Make sure there is an update to service library reference in the service wrappers/template once this PR is merged.
• Does an update need to be made to the engine?
• Was the new feature tested by unit tests?
• Was the new feature tested by a manual, end-to-end test?
