MedComCareCommunication profile
- Added requirement to include the element Communication.identifier. The identifier must be an uuid-v4.
- Clarified the use of practitioner roles for each payload.
MedComCareCommunicationMessageDefinition profile
- Added profile which describes the characteristics of a CareCommunication message.
MedComCareCommunicationProvenance profile
- Added profile, which inherits from MedComMessagingProvenance, to include a reference to the included payload in the MedComCareCommunication.
MedComCareCommunicationMessageHeader profile
- Added requirement to include a link to the MedComCareCommunicationMessageDefinition.
Restricted the element MessageHeader.focus from 1..2 to 1..1

Clarifications have been made in ClinicalGuidelines for application by @mbumedcom in #44
The Clarifications includes:- Section 4.2: Clarification of the description of a message segment
- Section 4.2.1: Updated description of formatting in a message text field
- Section 4.2.1.1: Addition of new section on copied or attached journal notes
- Section 4.4: Clarifications and added text about attachments, including name of author, as well as time of creation of the attachment
- Section 4.5.1: Use of IDs when responding to a MedCom message has been clarified
- Section 4.5.2: Handling of forwarding a correspondence message has been adjusted and clarified
- Section 4.5.3: Description of cancellation and correction is clarified
